Affiliate Form:
Name: The name of your site.
Rating: PG etc. We do not affiliate with sites that have a high rating.
Link: The link to your site.
Banner Code: For easy placement of your banner.
Placement: Where is our banner on your site? You must have our banner up before making an affiliate request.
Scrolling/Static: Is our banner scrolling or static? Your banner will be placed the same as ours.
